As a new patient you will pay 1 of 3 possible fees to join the clinic.
This is a one-time payment only, and after that all you will ever pay for is the cost of your treatment.
There are no monthly membership fees, no cancellation fees, and no monthly billing.

(Required for Testosterone) This is a one-time payment that covers patient enrollment, blood work/labs, medical charting, chart review, pre-appointment consultation, and your telehealth appointment with the Prescribing Physician.
If you have blood work less then 6 months old that includes Testosterone level, CBC, and Estradiol, please email them to us upon completion of this form for a $100 reduction of this cost.
New Patient Intake Fee for Weight Loss, Anti-Aging, or Peptide Treatments – $150
(Required for Weight Loss Treatments and Peptides) This is a one-time payment that covers your patient enrollment, medical charting, chart review, pre-appointment consultation, and your telehealth appointment with the Prescribing Physician.
If you’re seeking Testosterone or other hormone replacement, blood work is required, and you will need to select the $300 option.
Are You Transferring To Us From Another TRT Clinic ?
*If You’re Transferring To Us From Another TRT Clinic, The Fee Is $200 If Your Have Your Previous Labs.
*If you are transferring to us from another clinic and you have your most recent labs, we can expedite your intake process. Tranferring patients will pay is a one-time fee of $200. This fee covers your patient enrollment, medical charting, medical chart review, your pre-appointment consultation, and your virtual care / tele-health appoiontment with the Prescribing Physician. There are no additonal fees required of you.
Breakdown of The Intake Cost
Blood Work – $100
To begin hormone replacement therapy, we require labs. We will provide you an order# for the Labcorp or Quest nearest you. Walk-ins are welcome, and the process takes only a few minutes. If you have your own blood work, we will reduce the $300 fee to $200. You must upload your labs during the intake process, or email them to us at the time of sign up.
Physician Fee – $100
This one-time fee covers your telehealth appointment with the Prescribing Physician. *Prescriptions are good for as long as you continue treatment.
Clinic Fee – $100
The fee covers your application processing, enrollment, medical charting, lab and chart review, your pre-appointment telephone consultation with clinic staff, and your appointment with the Prescribing Physician.
